Do you have an Advance Care Directive? Now is a great time to think about your future health care, end of life and preferred living arrangements.

National Advance Care Planning Week (18 - 24 March 2024) is the perfect time to start a conversation about what matters most to you in terms of future health care preferences.

What is an Advance Care Directive?

There may be a time in your life when you can’t make your own decisions. This could be because of a sudden accident or illness. An Advance Care Directive empowers you to make clear legal arrangements for your future health care, end of life, preferred living arrangements and other personal matters.

An Advance Care Directive makes it easy for others to know what your wishes are if you are unable to make these decisions yourself. It can also give you peace of mind to know that your wishes are known and will be respected, if others need to make decisions for you.

Help with your Advance Care Directive

Residents on the Fleurieu Peninsula can book an appointment to get assistance in completing their own Advance Care Directive.

Appointments are available at the Victor Harbor Civic Centre, Yankalilla Library or Alexandrina Council offices (Goolwa and Strathalbyn). The one-on-one appointments are held with a Trained Peer Support Officer to you to navigate the Advance Care Directive Form.

Appointments usually take around 45 minutes, and you can book a follow-up appointment if you need further information.

The appointments are delivered by volunteers and provide information that is general in nature. If you require specific advice about the content, meaning and effect of the ACD Form you will need to seek support from professional services.
