Fleurieu Advance Care Directive Project

There may be a time in your life when you can’t make your own decisions. This could be because of a sudden accident or illness. An Advance Care Directive empowers you to make clear legal arrangements for your future health care, end of life, preferred living arrangements and other personal matters.

An Advance Care Directive makes it easy for others to know what your wishes are if you are unable to make these decisions yourself. It can also give you peace of mind to know that your wishes are known and will be respected, if others need to make decisions for you.

One-on-one appointments launched

The City of Victor Harbor, District Council of Yankalilla and Alexandrina Council have been working together to launch a new community service that will equip people with the information they need to complete their own Advance Care Directive.

Residents can now make a free appointment at the City of Victor Harbor, Yankalilla Library or Alexandrina Council offices (Goolwa and Strathalbyn) to get their Advance Care Directive underway!

The one-on-one appointments are held with a Trained Peer Support Officer to assist people to navigate the Advance Care Directive Form.

Appointments usually take around 45 minutes, and you can book a follow-up appointment if you need further information.

The appointments are delivered by volunteers and provide information that is general in nature. If you require specific advice about the content, meaning and effect of the ACD Form you will need to seek support from professional services.

How to book an appointment

Booking a one-on-one appointment is easy.

At the City of Victor Harbor Civic Centre (1 Bay Road, Victor Harbor), appointments are available on Mondays and Wednesdays, subject to availability. To find out the next available times, email localgov@victor.sa.gov.au or call 8551 0500.

Appointments can also be booked in Yankalilla or Goolwa. Simply use the contact details below based on where you would like your appointment:
